Goal:
Identify and collectively address prioritized community health issues
Strategies:
- Community health needs assessment and planning
- Increase the utilization of Health Compass Milwaukee as a continuous health needs assessment, planning, and impact measurement resource
- Identify, and when appropriate, align shared activities across health care and public health organizations to reduce duplication and maximize impact
- Coordinate 2024 cross-sector triannual community health needs assessment (CHNA)
- Community investments
- Continuously improve Shared Community Investment Fund and Health Improvement Fund pledges, allocation strategies, investments and impact
- Violence prevention
- Implement prioritized health care sector violence prevention and intervention initiatives
A ‘population health framework’ suggests that the health of an individual is affected by four health factors: health behaviors, access to, and quality of clinical care, social and economic factors and the physical environment. Recognizing that clinical care represents about only 20% of someone’s health, the Partnership members participate in key community initiatives that have a particular focus on low-income and vulnerable families — and have a significant impact on improving community health.
