Goal:
Collaboratively identify and address community health issues for underserved populations

- Community Health Needs Assessment and Planning
- Disseminate the 2024 CHNA and participate in plan alignment for collective health improvement initiatives
- Increase utilization of Health Compass Milwaukee and optimize its functionality through the inclusion of additional key community health data, such as mortality data
- Coordinate 2024 cross-sector triannual community health needs assessment (CHNA)

- Improve Shared Community Investment Fund allocation strategies, investments and impact and launch Mental Health Improvement Fund to facilitate increased pledges and a more targeted impact

A ‘population health framework’ suggests that the health of an individual is affected by four health factors: health behaviors, access to, and quality of clinical care, social and economic factors and the physical environment. Recognizing that clinical care represents about only 20% of someone’s health, the Partnership members participate in key community initiatives that have a particular focus on low-income and vulnerable families — and have a significant impact on improving community health.
