Goal:
Improve community health through cross-sector collaboration
Strategies:
- Improve health care for high-need populations defined by clinical, demographic, geographic and payer determinants
- Participate in cross-sector initiatives that influence community health, such as public safety, neighborhood and economic development, education, and public health collaborations
A ‘population health framework’ suggests that the health of an individual is affected by four health factors: health behaviors, access to, and quality of clinical care, social and economic factors and the physical environment. Recognizing that clinical care represents about only 20% of someone’s health, the Partnership members participate in key community initiatives that have a particular focus on low-income and vulnerable families — and have a significant impact on improving community health.
