The Partnership’s Priorities for the Year Ahead

Each year, the Milwaukee Health Care Partnership (MHCP) provides a synopsis of the activities its members will collectively sponsor,participate in, and monitor; under the consortium’s four priorities: coverage, access, care coordination and community health.  The Partnership’s FY19/20 Plan Priorities reflect the sponsored and supported initiatives that MHCP work teams will focus on, as well as the community partners integral to those efforts.

“This coming year, we will see intensified effort in our behavioral health strategies,” says Joy Tapper, MHCP executive director.  “The Partnership’s behavioral health provider steering committee will continue to support the key elements of our work plan, including advancing the Milwaukee Psychiatric Crisis Redesign, from study to implementation.  Those efforts include creation of community-based access centers in collaboration with Milwaukee’s FQHCs and the county Behavioral Health Division, supporting the development of a third crisis resource center in partnership with the Milwaukee Center for Independence, further preparations for new psychiatric emergency services, enhancing transition care management and post-discharge follow-up services for patients, and completing a roadmap for the implementation of trauma informed crisis services for children and adolescents.”

New in the FY19-20 plan is a homelessness / housing navigation initiative and increased emphasis on health information exchange under the care coordination priority, as well as the implementation of violence prevention strategies, in collaboration with the Office of Violence Prevention, under the community health category.
