MyHealthDirect Video Profiles ED to Health Home Program

Since 2009, more than 50,000 emergency department visitors have been referred to primary care health homes through a collaboration among Milwaukee’s adult health systems and more than twenty safety net clinics.  A key component in the program’s success is the appointment scheduling tool provided by MyHealth Direct, which helps care coordinators in emergency departments and community clinics secure primary care appointments for patients that are timely and close to home.
